Timestamped Audit Trail
Every edit, finalization, deletion and restore is permanently recorded with who did it and when, and the history can't be altered or erased. Unfinalizing, revising or deleting a report also asks for a reason, which is recorded with the event. A History panel on each report shows the full timeline, and the inspector's name and qualifications are embedded in the document.
NIST-Traceable Reference Standards
Log your reference instrument serial numbers and calibration dates. Autoclave certificates include a full NIST-traceability section out of the box.

Finalized Report Lock
Reports can be finalized to prevent edits — preserving the integrity of the record exactly as it was signed off. For corrections, users with permission can create a new revision (Rev B, Rev C…) that keeps the issued certificate exactly as it was, or unfinalize the report and edit it in place. Both are recorded in the report's history along with the reason given.

Frequently asked questions
How much does EquipQA cost?
EquipQA is free for teams right now. There are no hidden fees, no credit card required, and no usage limits. Paid plans for larger departments will be announced in advance, and existing users will always receive preferred pricing.
How is our data protected?
EquipQA is built with HIPAA-aware security practices. All data is encrypted in transit (TLS) and at rest. Access is scoped per organization — users outside your department cannot see your reports or equipment records. We do not share or sell any data.
Will EquipQA reports satisfy Joint Commission auditors?
EquipQA generates dated, signed PDF reports that include device identification, reference instrument serial numbers and calibration dates, measured values, pass/fail determinations, and a summary of findings. These reports are designed to meet the documentation expectations of Joint Commission EC.02.04.01 and related standards. We recommend reviewing your specific accreditation requirements with your compliance team.
Which equipment types are supported?
Currently supported: dental X-ray (scored against AAPM TG-175 or Health Canada Safety Code 30, chosen per report), dental radiation protection surveys, anesthesia vaporizers, anesthesia machines, steam autoclaves (including engineering certificates with NIST traceability), and ECG/EKG machines. Additional device types — including patient monitors, infusion pumps, and surgical equipment — are on the roadmap. Signed-in users can submit feedback to help prioritize what's built next.
Can my whole department use it together?
Yes. EquipQA supports multiple users under a single organization account with role-based access control. Administrators manage the account and can see all reports; technicians log results; and Paused accounts retain read access without the ability to modify records. There is no per-seat fee.

Choose the standard for each X-ray report — Score each dental X-ray report against AAPM TG-175 or Health Canada Safety Code 30 (2022). Limits, sources and criterion explanations follow your choice, and administrators can review and adjust each standard's limits in Settings. Existing reports are unchanged.
Fuller X-ray test records — Machine tests now record their raw readings and test conditions — including measured leakage, geometry and measurement mode. Every test uses a fixed set of statuses, now including "Not performed", and N/A requires a reason. Missing tests have been added, including apron and thyroid collar integrity.
Complete X-ray report record — Reports now capture the inspection type and scope, tester oversight and independence, an overall outcome with a corrective action for every failed item, film/digital receptor and viewbox/display QA, and the site's QC programme records. A report with a failed site QC verdict can no longer be signed off as "Acceptable for clinical use".
New: Dental Radiation Protection Survey — A dedicated report for the facility survey described in Safety Code 30 (B.5.2 and the Appendix III checklist) — room boundaries, operator position, signage, dosimetry, local rules and the protective-equipment inventory.
Sidebar grouped by equipment type — Report types are now grouped under headings such as Dental Radiology and Clinical Equipment, with the radiation protection survey listed alongside Dental X-Ray QA.
Criterion explanations for every device — Tap any test criterion on vaporizer, anesthesia machine, steam autoclave and EKG reports to see what it checks and how it affects the result — just like dental X-ray.
"Reports saved on this device" panel — See each locally saved report with its tool, testing date, last edit and upload status, and safely remove device copies. Work that hasn't uploaded yet is only removed after a warning, an offer to download a copy, and a second confirmation; a report open in any tab can't be removed.
More reliable syncing — Pending uploads now always finish: syncing no longer stalls after an unexpected error, when your connection returns (an expired sign-in is renewed automatically), or on a Safari storage error. The "pending upload" count no longer gets stuck, and the status badge reflects each report's real state — no alarming errors on finalized, read-only reports.
Each account's offline work kept separate — On a shared device, every account's locally saved reports are kept apart. Signing in as someone else locks the previous user's unsynced work instead of mixing it with yours.
Smarter handling of simultaneous edits — If a report was changed on another device, fields changed on only one side merge automatically. For true conflicts, a side-by-side panel highlights exactly what differs so you choose field by field instead of losing your work.
Restore deleted reports and equipment — Users with permission can view deleted reports from the report list and restore them. Equipment records open with a single click, deletion happens inside the record and can be undone, and each record keeps an edit and deletion history.
Annotations survive photo rotation — Rotating an annotated photo now turns the arrows, shapes and labels with it, and they stay fully editable.
Much faster certificates and cleaner PDFs — Engineering certificates generate many times faster, especially with a large company logo. Symbols such as Ω no longer print as garbled, widely spaced text.
Links, refresh and the Back button work everywhere — Every screen and report has its own link, refreshing reopens the same screen or report, and the browser Back button works — without losing unsaved edits. Open tabs are also prompted to reload when a new version is published.

Corrected certificates supersede instead of overwrite — Fixing a finalized report now creates a new revision (Rev B, Rev C, ...) while the originally issued certificate is preserved exactly as issued, with the reason for the correction recorded.
Deleted reports are recoverable, not gone — Deleting a report now requires a reason and can be undone from the History panel, instead of permanently erasing a compliance record.
Equipment recall dashboard — A new Equipment page lists every piece of equipment your team has serviced, with next-due dates, at-a-glance overdue/due-soon flags, and search & filters, so nothing slips past its recall interval.
Equipment records now created automatically — Equipment you test in a report is now automatically added to (or matched against) your Equipment Database, so the recall dashboard stays populated without extra data entry.
Fixed duplicate results in equipment search — The equipment picker could occasionally show what looked like the same device twice; search now always returns each match once.
Certificates generate without freezing the page — Generating an X-ray, autoclave, or vaporizer certificate — especially for reports with several devices — no longer risks a "page unresponsive" prompt while the PDF is built.
Reports now survive losing your connection — Saving a report always writes to your device first, so a dropped Wi-Fi signal or dead zone in a hospital basement never loses your work. Once you're back online, the report syncs to the server automatically in the background.
Sync status indicator — A status badge in the report toolbar shows whether your report is saved locally, syncing, or fully up to date on the server, with a tap-to-retry action if a sync attempt is waiting.
Photos upload independently and retry on their own — Each photo you attach syncs on its own in the background, so one slow or failed photo upload never blocks the rest of the report from saving.
Safer conflict handling — If a report was edited elsewhere while you were offline, you're now clearly warned and given the choice to keep your local version or load the latest one, instead of silently overwriting someone else's changes.
Warnings before you could lose unsynced work — Closing the tab or logging out while a report hasn't finished syncing now prompts a confirmation first.
Add EquipQA to your phone's home screen — iPhone and iPad users now get in-app instructions for adding EquipQA to their home screen for quicker access in the field.
